A care manager can visit your parent in the hospital; communicate professional-to-professional with the hospital discharge planner and physicians; explain your options for long term care; recommend specific facilities that meet your parent’s physical needs and financial resources; ensure that prescriptions are properly filled and dosage instructions carried out; ensure that follow-up care and medical appointments are in place and carried out; serve as an ongoing monitor/manager on your behalf – and keep you informed.
